To schedule an appointment with one of our physicians, please contact our office at (205) 930-2060 (ST.Vincent's Location) or  (205) 995-9909(119 location) or scroll down to schedule online.

Office visits are made by appointment only, Monday through Friday. Please schedule your appointment as far in advance as possible to allow for a better choice of day and time. If requesting an appointment online, please specify a day and time of day when you are available to come in.  The more flexibility in your schedule the greater the likelihood of our being able to offer you an appointment in a timely fashion. While we make every effort to honor your request, choices will also be dependent on your provider's schedule.

Please let us know the type of exam you wish to schedule, such as follow-up, yearly, company physical, or insurance exam. It is very important that you register with your full name as listed on your insurance card. Please notify us of all insurance or address changes. For your convenience, you can print and complete your registration forms prior to your visit. Please bring them with you to your appointment.

If you are unable to meet your scheduled appointment time, please call the office as soon as possible for rescheduling. Patients should arrive 15 minutes before their scheduled appointment. Late arrivals (in excess of 30 minutes past appointment time) may be asked to reschedule. We know that your time is valuable, and we will make every effort to honor our commitments.

Please bring the following on your initial visit with us:

• Your insurance card
• Your driver's license
• A list of current prescriptions and/or over-the-counter medications you are taking, including dosage and frequency
• Pertinent information regarding your medical history
